Separate phone reputation flags from observed reachability

A phone reputation result is a timestamped provider assessment, not a live call test. When an inactive-line warning conflicts with reported successful calls, inspect the persisted raw boolean and status, verify that lookup, submitted contact, and assigned driver refer to the same normalized number, and trace the UI predicate before deciding whether the fault is missing-field coercion, number mismatch, or provider disagreement. Check completion state separately: a warning can coexist with successful verification. A successful call remains reported evidence until independently corroborated.
