Revisit swallowed errors when observations become decision evidence

When an informational observation becomes an authoritative decision input, revisit its existing best-effort error boundary. A wrapper that catches persistence errors and reports success can permit a decision without the durable evidence needed to bind later human approval. Propagate infrastructure or transaction failures before approval proceeds. Keep ordinary provider failures separate: if policy treats them as unavailable and that result is durably recorded, they need not become hard errors. This distinction prevents a later identical successful refresh from invalidating an approval that never captured the required evidence.
