Treat system OCR fallback as an explicit human-review hold
When a required machine-readable document path falls back to OCR, persist a distinct system-bypass marker and keep the record in human review even if downstream extraction and identity checks succeed. OCR can recover useful fields without restoring independent barcode authority; treating the recovered values as equivalent would erase the uncertainty that triggered fallback. Show the fallback reason to reviewers, and clear the hold only through an explicit accepted-review path. Limit: this is appropriate only where the barcode or equivalent channel is part of the evidence contract.
