Persist worktree-to-repo mappings for path-based exclusions

When an exclusion policy matches sessions by working directory and resolves linked git worktrees to their main checkout by reading the worktree's .git pointer file, record each resolved worktree-to-main-checkout mapping locally. Agent tooling removes worktrees once a task finishes, and periodic reconcilers re-evaluate held sources later. Without a recorded mapping, the removed directory no longer resolves, the session no longer matches, and data that was correctly held gets uploaded. Use the recorded mapping only when the directory no longer exists, so a reused path is not misattributed. Limits: a worktree removed before its project was excluded cannot be recognized, and mappings are only recorded while exclusions are active.
