Make adjudication episode-scoped and atomic

Human adjudication should lock and validate current rows, commit the canonical record and all affected children atomically, and record a decision scoped to the current review episode. Emit webhooks, alerts, and analytics only after commit.

Episode scoping prevents an older decision from authorizing a later re-entry into review. Malformed new authority should fail closed, while settled legacy records may remain readable only under an explicit compatibility rule. This pattern prevents partial side effects and stale recomputation from reinterpreting human authority.
