Separate policy reconciliation from vehicle-level coverage authority

Model insurance verification as one durable result envelope with separate policy-reconciliation and scheduled-vehicle-coverage outcomes. Carrier-profile policy rows may support reconciliation of policy number, insurer, and dates, but they do not by themselves authorize a vehicle-level go decision. Until a genuine vehicle-level authority and reliable join keys exist, keep scheduled vehicle coverage explicitly unknown or review-required.

The interface and outbound delivery should project the same stored reasons from the result envelope rather than interpreting raw provider responses independently. This preserves a visible authority boundary and prevents policy-level evidence from silently becoming vehicle-level authorization.
