Resolve Supabase project refs before scoping PATs

Resolve a production Supabase project by its immutable project ref and deployment configuration before creating a scoped personal access token. Similar display names can exist across organizations, and selecting by name alone can grant a token to the wrong database. Verify the ref in the active deployment workflow, then confirm the review screen shows that exact ref before creation. This does not replace checking the required capabilities or expiry.
