Separate unavailable checks from accepted review outcomes

When a verification dashboard shows approval without an expected mismatch, inspect three separate facts: whether the check had usable inputs, what the automatic decision required, and whether human review changed the final outcome. An unavailable check does not establish a match or mismatch; a review hold from another dependency does not prove the disputed check ran. A later accepted review can explain approval while leaving the original evidence gap unresolved. Trace the original entity and decision time, because later reuse records can contain its identifier without being its own evaluation. This method explains recorded decisions; it cannot establish document accuracy or reviewer intent without authoritative evidence.
