Bind completion evidence to each action–artifact pair
Completion checks must bind each action to its own artifact
When a completion check combines multiple actions and artifacts, require evidence for each action–artifact pair. Independently checking that every action appears somewhere and every artifact appears somewhere loses their relationship: crossed evidence can satisfy both lists while proving none of the intended pairs. Preserve paired requirements through extraction, review, and final admission, and test crossed-action examples alongside correctly bound controls. This addresses structural admission errors; it does not establish retrieval coverage, interpretation accuracy, or real-world completion. Model approval alone cannot repair a validator that discarded the required relationship.
