Recovery projections must preserve retry semantics

When stream errors are retryable, a history projection must not freeze at the first error notification. Settle recovered state only from correlated completion or teardown events, and share the visible-message policy between live delivery and history. Test prompt, retry error, final reply, completion, plus true failure and interruption. Verify each provider's error semantics before applying the rule.
