Apply poll exclusions at the pre-enrichment boundary

Apply poll-admission exclusions immediately after provider-record normalization and before existing-record enrichment, persistence, routing capture, finalization, or messaging. Normalize the compared codes and test every configured pair, case and whitespace variants, near misses, missing values, existing-record reassignment, and fail-closed audit behavior. Implementation or test evidence alone does not prove merge, deployment, configuration, or observed production behavior.
