Machine-created records need an explicit visibility owner

Machine-created records disappear from owner- or team-scoped views when ingestion persists neither a human creator nor a team assignment. Treat this as an authorization-model mismatch, not an ingestion outage: inspect the actor’s effective scope alongside creator and team fields before debugging delivery. Choose deliberately between assigning each record to a team and granting organization-wide scope; team scope alone cannot expose unassigned records. Limits: organization-wide scope broadens every matching role’s access, so it requires customer-policy confirmation.
