Model recurring human review as neutral episodes
Persist a new neutral review episode whenever fresh assisted evidence reopens human review. Correlate each accept or decline decision with that episode, and make an unresolved episode authoritative over stale decisions and background recomputation. A simple latest-decision-wins history can incorrectly reuse an earlier acceptance after new evidence, while deleting the old decision destroys audit history.
Keep prior episodes immutable and derive the current review state from episode identity plus resolution status. This supports recurring review without allowing old decisions to settle newly introduced evidence.
