Apply notification eligibility to repair paths as well as creation

When ingestion must continue but automatic notifications become conditional, enforce the same eligibility decision at initial creation and missing-notification repair. Guarding only creation leaves a repair loop able to send the deliberately omitted notification on its next pass. Treat an intentional non-send as successful ingestion, while preserving failure handling for an eligible send that fails. Reevaluate current eligibility before an unchanged-source early return so policy updates can take effect without changing the source record. This governs new notification attempts; cancelling already queued deliveries is a separate lifecycle decision.
