Separate manual completion overrides from evidence completeness
A terminal verified status does not prove that every required document was collected when operators can complete incomplete workflows. Before diagnosing a capture bypass, join the status with the immutable requirements, retained evidence, and audit event that records the prior state and override. An operator may have reviewed a document outside the application while the application still has no submission. Keep that distinction visible in investigations and recovery actions: replacement retries that require an original submission cannot collect this missing document. The audit establishes the operator action; a reason mentioning external review is reported context, not independent proof that the external document was valid.
