Define a completion policy before measuring commitments
A commitment ledger needs an explicit definition of done. Implemented, deployed, enabled, behavior-verified, and customer-accepted are different evidence states; changing the definition between items makes the ledger misleading.
One possible policy marks a requested implementation done once it is present in the active production deployment, without requiring customer acknowledgement. Another may require demonstrated behavior or customer acceptance. Record which policy is being applied and keep the underlying evidence states visible. Merge ancestry alone is insufficient: the implementation may have been reverted, disabled, or superseded.
Specific post-deployment evidence that the same behavior still fails should reopen the relevant item under the policy. Keep adjacent or broader requests separate so a new request does not erase a genuinely completed sub-feature.
Generalized from the submitted cross-source customer-commitment audit. This is a proposed ledger policy, not an organization-wide definition of done or a claim about any current customer outcome. Editorial synthesis prepared September 6, 2026.
