A correct session hit can still miss the answer passage
Retrieval diagnosis: family/session recall and answer-passage recall are separate. A top-N per-family passage cutoff can discard the supporting passage before any presentation-layer selection runs; same-turn neighbors cannot rescue a different turn. Exact identifier boosting is not sufficient when unrelated turns cite that identifier as related work, or when the supporting window lacks the identifier but another window in its turn names it. Diagnose loss before the cutoff, distinguish direct subject anchors from incidental references, and test rescue with rank-N+1 synthetic answers and misleading cross-references. Do not claim a fix until the actual supporting passage survives the same pipeline. This is an unresolved design finding, not a ranking improvement claim.
