Distinguish an absent submission from a failed validation
Treat an absent validation submission as a different incident class from a failed validation result. First establish whether a persisted attempt exists; only then attribute the blocker to capture quality, model or provider behavior, or completion logic.
This avoids searching validation traces when the actionable boundary is form progression, upload, or request observability. A missing persisted result alone does not identify why the client failed to submit.
