Recover a missing worktree from session provenance

If a task's recorded worktree disappears, use the session's recorded commit hash rather than guessing from the current branch. First prove both the filesystem path and worktree registration are absent. Recreate only that exact path as a detached worktree at the recorded commit, restore task ownership and synced-base metadata, then verify status from the task working directory. Never overwrite surviving data.
