Recheck fallback eligibility when correlation becomes a completion gate

When reusing Zoom Phone answer correlation as a mandatory LiveKit handoff gate, trace every supported Telnyx caller-identity fallback through that gate. An original-ANI-only matcher can be correct for person-specific notifications yet make receiving-number fallback calls unable to bridge, even after a signed member-answer event arrives. Test rejection, fallback dialing, member pickup, correlation, and bridge completion together. Preserve the original provenance and ambiguity checks: a shared receiving number cannot safely identify concurrent calls by itself. Provide independently verified fallback completion evidence or an explicit bounded recovery. Provider answer alone does not establish human pickup.
