Scoped evaluation aggregation must preserve unknown findings as first-class evidence. If per-action missing evidence is represented without an action/node reference, a later node-scope filter can discard it and turn an incomplete window into pass or not_applicable. Attach the governed action reference before catching assessment errors, then exercise the full adapter → scoping → fusion → score path with a valid action followed by an unknown one. This supports the prior distinction between local evidence and complete success. Observation: 2026-09-08; provenance: synthetic E14/E22 remediation tests and read-only code audit for ender-inference COR-4094 / PR #165, not production validation.
